How do no-claims discounts work on health insurance?

The short answer

Many policies run a no-claims discount scale: each claim-free year moves you up the scale and cuts the claims-related part of your premium; claiming moves you down. Scales, steps and maximum discounts differ by insurer and are set out in the policy documents. The discount affects only part of the renewal price — age and medical inflation still apply.

Reading a scale before you rely on it

A no-claims discount is a mechanical scale: defined claim-free years move you to defined discount levels, and a claim moves you back down by a defined number of steps. Published scales differ by insurer — Bupa’s Bupa By You scale, for example, runs to 14 discount levels with a maximum 70 per cent discount, applied to the net subscription rate excluding Insurance Premium Tax, and calculated separately for each person on a multi-person policy (source 3). Other insurers use their own scales, and some protect the discount for a first claim or below a cost threshold. Two points are commonly missed: the discount applies to the claims-related component, not to age or book-level increases, so premiums can rise while the discount holds; and the scale rules sit in the policy wording, not the brochure, so read them before assuming a claim is “not worth making”.

Before letting a small claim decision be driven by the discount, ask the insurer what the claim would cost you in scale steps over the following years — in writing. Does a no-claims discount protect my whole premium from rising?
No, and this is a common misunderstanding. The discount applies to the part of the premium linked to your own claims history, not to the whole price. Age moving up a band and medical inflation across the insurer’s whole book can still push the overall premium up even while your personal no-claims discount stays at its maximum level. So a steady or rising price at renewal, despite a clean claims record, does not necessarily mean the discount has stopped working — it usually means other pricing factors have moved instead.
Is it always worth avoiding a small claim to protect my discount?
Not automatically — it depends on the scale and the size of the claim. Some insurers protect the discount for a first claim or below a certain cost threshold, so a small claim may not move you down the scale at all. Where it would cost scale steps, the right comparison is between the value of the claim now and what those steps would add to your premiums over the following years. Asking the insurer directly, in writing, what a specific claim would cost in scale terms is more reliable than guessing from the brochure.
Does a no-claims discount transfer if I switch insurer?
Not automatically, and this should never be assumed. Each insurer runs its own scale with its own steps and maximum discount, so a position built up with one insurer does not simply carry across to another on the same terms. Some insurers may take a previous no-claims history into account when quoting, but this needs to be confirmed in writing as part of getting a new quote. The safer approach is to ask directly how a switch would affect your discount position before deciding to move.
